RE: New Coverage for Audiology Cochlear Implant

Effective July 1, 2025, the Alabama Medicaid Agency will initiate coverage for procedure code L8694 (Auditory osseointegrated device, transducer/actuator, replacement).

Procedure code L8694 will:

• Require prior authorization
• Be limited to recipients ages 0 to 20 years of age
