If you feel that your Medicaid paperwork has been lost in the recent Butler County Health Department fire, please call the Recipient Call Center at 1-800-362-1504.

Medicaid paperwork may be your application, paper renewals, Recipient Change Form, 
Medicare-Savings Program (MSP) application, etc.
