PCP Groups Required to Complete New Agreement
The ACHN anticipated start date is October 1, 2019. Primary Care Physicians (PCPs) who want to receive, without delay, bonus and participation payments in conjunction with the state’s planned ACHN program must sign a Participation Agreement with an ACHN Entity.
The Participation Agreement Between the ACHN Entity and the Primary Care Provider Group
• Replaces the current Health Home agreement
• Is an agreement between the ACHN and the PCP group
• Only one Participation Agreement with an ACHN Entity is required for an individual PCP practitioner or
a PCP group to participate with all ACHN Entities.
• This agreement details the responsibilities of the PCP or PCP group to actively participate with the
ACHN. These responsibilities are:
a. Participate, as needed, in the ACHN’s Multidisciplinary Care Team and the development of
individualized and comprehensive care plans;
b. Over a twelve (12) month period, participate, in person, in at least two (2) quarterly Medical
Management Meetings and one webinar/facilitation exercise with the ACHN’s Medical Director.
(Attendance requirements can be met by having one PCP or Nurse Practitioner/Physician Assistant
from the group attend);
c. Participate in ACHN initiatives centered around quality measures; and
d. Review data provided by the ACHN to help achieve Agency and PCCM-E quality goals
The ACHN should contact your practice regarding this agreement in the near future or you may contact the ACHN in the region(s) where you practice.
The ACHN MUST notify Medicaid of the status of PCP agreements on or before July 1, 2019. Please submit your PCP agreement to the ACHN before the above date to ensure there is no delay in receiving participation and bonus payments.
The following provider types are eligible to participate with the ACHN as a PCP:
• Family Practitioners
• General Practitioners
• Pediatricians
• Internal Medicine
• OB/GYNs
Any provider not listed above (e.g., physician assistant, nurse practitioner) but practicing within a PCP group is also eligible to participate with the ACHN.
When in the best interest of a patient (e.g., children with special health care needs), a nontraditional PCP specialty may participate in the ACHN program as a PCP. Physicians with other specialties may be considered for PCP participation if willing to meet active participation requirements.
